The questionnaires are to be obtained from the Reichsdruckerei, Berlin SW 68, Oranienstr. 91. (22) Responsible for the issue of questionnaires and supplementary questionnaires, and for deciding on admission to the German People's List, is the branch of the German people's list, in whose district the Volksdeutsche was resident on 1 December 1939. In the case of persons who at that time had their residence outside of the integrated Qst areas in the Great German Reich or abroad, the branch of the German People's List is responsible for the decision in whose district the German people had his last residence in the integrated eastern areas; if he has never had a residence there, the division of the Deutscher Volksliste in the city of Poznan is responsible. Questionnaires and supplementary questionnaires may also be issued abroad by the German consular services to persons who lived outside the integrated eastern territories on 1 December 1939, by any authority responsible for issuing nationality certificates. The questionnaires and supplementary questionnaires are to be issued in 2 pieces each, to residents of the integrated eastern areas in 3 pieces. Of this, 1 remains with the branch of the German People's List responsible for the decision, while the 2nd piece, if the admission into the German Volksliste is ordered, is sent to the Reichsssicherheitshauptamt in Berlin Sw l1, Prinz- Albrecht-Str.8, to which also the President of the Government in Poznań has to submit the second pieces of the questionnaire received by him on the basis of the circular of 25 November 1939; the 3rd piece is in the annexed eastern regions of the NSDAP. Persons who have already submitted questionnaires on the basis of the circular notice dated 25 November 1939 shall normally no longer be required to issue further questionnaires. (23) C8S10
