yuffr. wm 24.9. 4941 r2B.Jiljin. wn THE OBERLANDRAT Jitschin_..-.., the_16.Sept._194_1. Sequence $-6-5. E To Miss Anna Čerwenka 2 in Libish No.l9 Post_Neratowitz. Subject: Change of name. Reference: Your letter of 9.7.l940 addressed to the National Authority in Prague Annexes: 2 I would like to repeat the request for a change of the surname -xxtxdexekxXuomamexosx- using the accompanying forms. I would ask you to fill in the forms carefully and to provide the documents described in them in a complete manner. As a new name, the German name of an ancestor (parent or grandparents) and, if necessary, the german name of the wife and their ancestors will be considered. However, it may also be possible to choose a name which calls to the previous name or which represents the German translation of the previous one. Some collective names, which are common everywhere (fisherman, Krause, Lehmann, Müller, Schmidt, Schulz, etc.), as well as double names may not be chosen. Also the interpretation of the spelling of a non-German name is to be omitted (Hudeček in Hudetschek or Hořický in Horschitzki). In such cases, only a complete change of name with a name of purely German origin is possible. If there are still questions of doubt, I would like to speak personally to my department. On behalf of: Government Inspector. Form No. 1 Böhmisch-Mährische Verlags- und Druckerei-Gesellschaft m. b. H., Prag II., Herrengasse 8.
